2025 CPT code 80400

ACTH stimulation panel for adrenal insufficiency. This panel must include cortisol (82533 x 2).

Code 80400 is for the laboratory component of the ACTH stimulation test. The administration of the synthetic ACTH is reported separately using codes such as 96360, 96361, 96372-96375. If the laboratory does not perform both cortisol assays, the individual cortisol code (82533) should be used instead of the panel code. If additional tests are performed, those should be reported separately.

Modifiers may be applicable in certain situations. Modifier 91 is used for repeat laboratory tests. Modifier 90 indicates that the test was performed by an outside laboratory.

Medical necessity for 80400 is established when there is clinical suspicion of adrenal insufficiency or other adrenal disorders. Symptoms like fatigue, weight loss, low blood pressure, or signs of an adrenal crisis justify the test.

The physician orders the ACTH stimulation test and interprets the results to diagnose adrenal insufficiency or other related conditions. The laboratory personnel perform the cortisol assays.

In simple words: This is a blood test that checks how well your adrenal glands work. It involves getting two blood samples to measure cortisol levels, one before and one after you receive an injection of a hormone that stimulates your adrenal glands. This helps doctors figure out if your adrenal glands are producing enough cortisol.

This code represents a complete ACTH stimulation panel used to assess adrenal function, specifically in cases of suspected adrenal insufficiency. The panel includes two cortisol tests (82533) performed before and after the administration of synthetic ACTH to measure the adrenal gland's response.

Example 1: A patient presents with symptoms of fatigue, weight loss, and low blood pressure. The physician suspects adrenal insufficiency and orders an ACTH stimulation test (80400) to evaluate adrenal function., A patient with known Addison's disease undergoes routine ACTH stimulation testing (80400) to monitor the effectiveness of their current medication regimen., A patient experiences symptoms suggestive of an adrenal crisis. An ACTH stimulation test (80400) is performed as part of the diagnostic workup to confirm the diagnosis and guide treatment.

Documentation should include the patient's medical history, reason for the test, time of ACTH administration, and time of blood sample collections. The interpretation of the cortisol results in relation to the ACTH stimulation should also be documented.
