Urinalysis; qualitative or semiquantitative analysis of urine, excluding immunoassays.

Refer to the current CPT guidelines for urinalysis coding.Pay special attention to the rules for selecting between analyte-specific codes and the unlisted procedure code (81099).

Modifiers may apply depending on the circumstances of service.Examples include modifiers 90 (Reference laboratory), 91 (Repeat test), 59 (Distinct procedural service), and others as clinically indicated.

Medical necessity is determined by the ordering physician based on the patient's clinical presentation, history, and suspected diagnosis.The test must be deemed appropriate to aid in diagnosis or management of a condition.

The laboratory analyst is responsible for performing the technical aspects of the test, including specimen handling, analysis, and reporting results. The ordering physician is responsible for medical decision-making based on the results.

IMPORTANT:If an analyte-specific code exists within the Chemistry (82009-84830) subsection, that code should be used instead.Code 81099 (Unlisted urinalysis procedure) can be used if no analyte-specific code is available.
