The clinical responsibility lies with the consulting pathologist who performs the intraoperative cytologic examination to provide immediate diagnostic information to the surgeon.

IMPORTANT:This code is always used in addition to code 88331 (first tissue block, with frozen section(s), single specimen) or 88333 (cytologic examination [eg, touch prep, squash prep], initial site).It should not be used with codes 88160-88162 (non-intraoperative cytologic examination) or 88172 (intraprocedural cytologic evaluation of fine needle aspirate).

The pathologist performs the initial cytologic analysis (88333) and then assesses two additional samples from distinct locations within the lung (88334 x 2).

Detailed operative report indicating the need for intraoperative consultation, pathologist's report specifying the number of sites examined, and the number of slides prepared.Specific location of each examined site should be noted.
