2025 CPT code 93226

External electrocardiographic recording up to 48 hours by continuous rhythm recording and storage; scanning analysis with report.

Code 93226 is reported only for the scanning analysis and report generation portion of the Holter monitoring service. It should not be reported with 93224 (global Holter monitoring service).

Modifier 59 may be necessary to distinguish 93226 from the global Holter code 93224 when performed as a distinct procedural service.

Medical necessity for Holter monitoring must be supported by documentation of symptoms suggestive of cardiac arrhythmias, such as palpitations, dizziness, syncope, or chest pain.

A qualified healthcare professional, such as a technician or nurse, is responsible for performing the scanning analysis and generating the report. The physician or other qualified healthcare professional is responsible for reviewing and interpreting the report.

It doesn't include hooking up the device or the doctor's interpretation of the results.

This code represents the technical analysis and reporting of data from a Holter monitor, which continuously records a patient's ECG for up to 48 hours. It involves downloading the recorded ECG data from the Holter monitor, analyzing the data for any abnormalities in heart rate or rhythm, and generating a report of the findings. This code does not include the connection and disconnection of the Holter monitor (93225) or the physician's review and interpretation of the report (93227).

Example 1: A patient experiencing intermittent palpitations undergoes a 48-hour Holter monitoring. Code 93226 is reported for the analysis and report generation of the recorded ECG data., A patient with a history of syncope has a Holter monitor placed to detect any underlying arrhythmias. Code 93226 covers the processing of the ECG recording and preparation of the report detailing heart rhythm patterns., A patient recovering from a myocardial infarction undergoes Holter monitoring to assess for any recurrent ischemia or arrhythmias.Code 93226 is used for the analysis of the Holter data and creation of a report.

Documentation should include the date and time of the recording, the reason for the Holter monitoring, the duration of the recording, a summary of the findings from the scanning analysis, and any relevant patient symptoms reported during the monitoring period.
