Medical necessity for a 93245 is established by the presence of symptoms suggestive of intermittent or prolonged cardiac arrhythmias, or the need to monitor the effectiveness of cardiac medication or treatment for previously diagnosed arrhythmias, which cannot be adequately assessed using shorter monitoring periods.

The clinical responsibility includes instructing the patient on the use of the device, applying the electrodes and monitor, retrieving the data, reviewing the initial analysis from the processing center, interpreting the results, and reporting the findings to the patient.

IMPORTANT:This code should not be used in conjunction with codes 93246, 93247, and 93248, which represent component parts of this global service.If only some of the components are performed, those individual component codes should be reported.
