Peri-procedural in-person device evaluation and programming of an implantable cardioverter-defibrillator (ICD) system before and/or after a surgery, procedure, or test.

Medical necessity is established by the need for peri-procedural evaluation and programming of the ICD to ensure optimal device function before, during (if applicable), and after a surgical, other procedural, or diagnostic intervention. This is particularly relevant in situations where the procedure may interfere with the device’s operation or when symptoms suggest malfunction.

The physician or other qualified healthcare professional is responsible for the in-person evaluation, interrogation, analysis, and programming of the ICD system. This includes reviewing stored data, assessing device function, and making necessary adjustments to the device’s settings to ensure proper operation before, during (if applicable), and after the procedure.

IMPORTANT:Use in conjunction with 0696T if body surface-activation mapping is performed.Do not report with 93260, 93261, 93282, 93283, 93284, 93289, 0408T, 0409T, 0410T, 0411T, 0414T, or 0415T.

Example 1: A patient with an ICD is scheduled for cardiac ablation.Prior to the procedure, 93287 is performed to ensure the ICD is programmed to safely interact with the ablation equipment (e.g., turned off or reprogrammed during the procedure to prevent interference). After the procedure, 93287 is repeated to restore the device to the patient’s optimal settings., A patient requires an elective surgical procedure that may interfere with the proper functioning of their ICD. Before the surgery, the physician performs a peri-procedural evaluation (93287) and re-programs the device to mitigate any potential interference, ensuring patient safety. After the surgery, the physician performs another peri-procedural evaluation (93287) and re-programs the device to its optimal settings., A patient experiences symptoms suggestive of ICD malfunction.An in-person interrogation and programming evaluation (93287) is performed to assess the device function, review the stored data, and modify the settings to restore optimal function.
