2025 CPT code 93452

Left heart catheterization, including left ventriculography, imaging supervision, and interpretation.

Consult the official CPT manual for the most current coding guidelines, including those related to cardiac catheterization and associated procedures.Note that 93452 does not include certain angiography or injection procedures which may need to be reported separately.

Modifiers 26 (professional component) and TC (technical component) may apply depending on who performs which part of the service.

Medical necessity is established by clinical findings indicating the need to evaluate left ventricular function, anatomy, and pressure gradients. This may include symptoms (e.g., chest pain, shortness of breath), abnormal findings on electrocardiogram or echocardiogram, and evidence of valvular heart disease or cardiomyopathy.

A cardiologist or other qualified physician performs this procedure. The physician inserts the catheter, guides it into the left heart chambers using fluoroscopy (real-time X-ray), injects contrast dye if needed, monitors hemodynamics, interprets images, and prepares a report.

IMPORTANT:Do not report 93452 in conjunction with 93453, 93458-93461, 0408T, 0409T, 0410T, 0411T, 0414T, 0415T.Do not report 93452 in conjunction with 33418, 0345T, 0483T, 0484T, 0544T, 0545T, 0643T, for diagnostic left heart catheterization procedures intrinsic to valve repair, annulus reconstruction, or left ventricular restoration device implantation.

In simple words: This medical procedure involves inserting a thin tube (catheter) into a chamber of the heart to examine its function and structure.A special dye might be used to help take pictures. A doctor monitors the process and reviews the images to assess heart health.

This CPT code encompasses the procedure of left heart catheterization. It involves catheter placement within the left-sided (systemic) cardiac chambers (left ventricle or left atrium). The procedure includes left ventricular/left atrial angiography, along with imaging supervision and interpretation. When performed, intraprocedural injections for left ventriculography are included.Note that this code does not include contrast injections and imaging for separately identified imaging procedures using other codes.

Example 1: A patient presents with symptoms suggestive of mitral valve disease.Left heart catheterization with ventriculography is performed to assess left ventricular function and identify the extent of valvular disease., A patient with suspected hypertrophic cardiomyopathy undergoes left heart catheterization to evaluate left ventricular hypertrophy and assess pressure gradients within the left ventricle., A patient post-myocardial infarction presents with persistent chest pain and reduced ejection fraction.Left heart catheterization is performed to assess ventricular function and determine the presence of significant coronary artery disease requiring intervention.

Complete patient history and physical exam, indication for the procedure, consent form, images from the procedure (angiograms, hemodynamic tracings), and the final report with interpretations.

** Always refer to the most recent CPT coding guidelines and payer-specific policies for accurate coding and reimbursement.Understanding the anatomical variations of the heart is crucial for correct code selection, particularly in congenital heart disease.
