2025 CPT code 93586

Venography of the coronary sinus during heart catheterization for congenital heart defects.

Code 93586 is an add-on code and should not be reported alone. It should always be reported in addition to the appropriate primary procedure code for the heart catheterization.

Modifiers may be applicable to this code.

Medical necessity for venography of the coronary sinus should be established by documenting the clinical need for the procedure in the context of the patient's congenital heart defect.

The physician performing the heart catheterization is responsible for placing the catheter in the coronary sinus, injecting the contrast dye, and supervising and interpreting the venography images. The physician also prepares a report documenting the findings of the procedure.

IMPORTANT:Use 93586 in conjunction with primary procedure codes 93593, 93594, 93596, or 93597. Report 93586 only once per session.

In simple words: During a heart catheterization procedure for congenital heart defects, a special x-ray is taken of the coronary sinus, a large vein in the heart.This involves placing a small tube into the vein and injecting dye to make the vein visible on the x-ray. The doctor then interprets the images.

Venography for congenital heart defect(s), including catheter placement, and radiological supervision and interpretation; coronary sinus. (List separately in addition to code for primary procedure.) (Use 93586 in conjunction with 93593, 93594, 93596, 93597.) (Report 93586 once per session.)

Example 1: A pediatric patient with Tetralogy of Fallot undergoes right and left heart catheterization (93597) to assess the severity of the defects. Venography of the coronary sinus (93586) is performed during the catheterization to evaluate the coronary venous anatomy., An infant with transposition of the great arteries undergoes a combined right and left heart catheterization (93597) prior to corrective surgery.Venography of the coronary sinus (93586) is performed to evaluate the venous drainage and assist in surgical planning., A child with an atrial septal defect undergoes right heart catheterization (93593) to assess the size and location of the defect.Venography of the coronary sinus (93586) is performed to rule out any associated coronary sinus anomalies.

Documentation should include the indication for the venography, a description of the catheter placement and injection procedure, the interpretation of the venographic images, and any associated findings.
