A qualified healthcare professional (physician, psychologist, or other qualified provider) administers and interprets the standardized instrument and generates a report documenting findings.

IMPORTANT:May be used in conjunction with other codes such as 96110 (Developmental screening) or other codes for additional assessment as clinically indicated.Do not report with 97151-97158 or 0362T, 0373T.
