Medical necessity for RTM must be supported by documentation demonstrating that the remote monitoring is necessary for effective management of the patient's musculoskeletal condition.The chosen monitoring method should align with the patient's individual needs and the clinical goals of treatment.It should be expected that the information gathered through RTM will meaningfully impact the patient's care plan.
A physician or other qualified healthcare professional orders the RTM service. The reporting provider then supplies and sets up the device(s), educates the patient on its use, and ensures data transmission occurs as scheduled.

Documentation should include the medical reason for RTM of the musculoskeletal system, the type of device(s) supplied, patient education provided, the frequency of data recordings and/or alert transmissions, and the total duration of monitoring during the 30-day period.Any changes to the treatment plan based on the RTM data should also be documented.
