The dressing is applied in the outpatient burn clinic and the patient is instructed on how to change the dressing at home., Following a surgical debridement of a large, non-healing ulcer on the leg, measuring more than 48 square inches, a sterile hydrogel dressing (A6244) is applied to protect the wound and facilitate healing. The patient is discharged home with instructions for dressing changes., A patient with a skin graft donor site exceeding 48 square inches requires a sterile hydrogel dressing (A6244) to promote healing and minimize discomfort. The dressing is applied in the operating room and the patient is provided with instructions for home care.

Documentation should include the size of the wound, the type of dressing used, and the medical necessity for the dressing, such as the presence of a burn, ulcer, or skin graft donor site. If the dressing is applied to an extremity, the left (LT) or right (RT) modifier should be appended. If furnished in conjunction with a surgical dressing, modifier AW should be used.
