Prolonged home or residence evaluation and management service(s) beyond the total time for the primary service.

This code is an add-on code and must be used in conjunction with the appropriate primary home or residence E/M service code (e.g., 99345, 99350). Each unit of G0318 represents 15 minutes of prolonged service time beyond the primary service. Report G0318 only when the prolonged time is spent by the same physician or qualified healthcare professional who provided the primary service. The time counted towards prolonged services includes both direct face-to-face time with the patient and time spent on the same date of service performing non-face-to-face activities directly related to the patient's care.

Medical necessity for prolonged services must be clearly documented. The additional time should be justified by the complexity of the patient's condition and the intensity of the services provided. Simply exceeding the usual time for a visit does not automatically justify the use of G0318.

The physician or qualified healthcare professional provides prolonged direct patient contact or spends additional time on the same date of service for non-face-to-face activities related to the home or residence E/M service.

The primary service code is billed, with G0318 added to account for the prolonged time spent beyond the typical time for a 99350., A physician assistant spends an hour with a new patient during a home visit involving wound care and patient education. Due to the complexity of the wound care, additional time was spent above the typical time allotted for a 99345. G0318 is used to bill for the additional time.

Documentation should support the medical necessity of the prolonged service time, specifying the total time spent on the date of service for both direct and indirect patient contact related to the primary E/M service, including a description of the activities performed during the prolonged service time.
