Home infusion therapy for antibiotic, antiviral, or antifungal drugs, including administrative, pharmacy, and care coordination services (excluding drugs and nursing visits).

Do not use S9494 with home infusion codes for hourly dosing schedules (S9497-S9504). Bill separately for the specific drugs administered using the appropriate HCPCS code (or J3490 with the NDC number if a specific code is unavailable) and for nursing services (e.g., CPT codes 99601, 99602). Modifier SH can be used for additional antibiotics administered on the same day. Certain insurances and Medicaid use S codes, but Medicare does not. Check with individual payers for reimbursement requirements.

Modifier SH may be applicable when billing for multiple antimicrobial drugs administered on the same day.

Code S9494 is used for the overall coordination of the home infusion therapy, while separate codes are used for the antibiotic itself and any nursing visits for administration., A patient with a compromised immune system requires long-term antiviral therapy administered at home. S9494 covers the administrative, pharmacy, and coordination services, excluding the antiviral drug and nursing services which are billed separately., A patient receives antifungal infusions at home for a systemic fungal infection. S9494 is billed for each day of the home infusion therapy services, separate from the antifungal medication and any necessary nursing visits.

Documentation should include the prescription for the infused medication, the plan of care for home infusion therapy, and records of all services provided, including administrative, pharmacy, and care coordination. Nursing visit documentation should be recorded separately. If applicable, documentation supporting the medical necessity of home infusion should also be included.
