If hypothyroidism is due to a drug or toxin, code the poisoning first (T36-T65 with fifth or sixth character 1-4). Use an additional code for adverse effect, if applicable, to identify the drug (T36-T50 with fifth or sixth character 5).This code excludes iodine-deficiency related hypothyroidism (E00-E02) and postprocedural hypothyroidism (E89.0).

Medical necessity is established by the presence of signs, symptoms, and laboratory findings consistent with hypothyroidism, causally linked to a medication or exogenous substance.The documentation should support the need for treatment and monitoring to restore thyroid function and alleviate symptoms.

Clinicians should evaluate patients for symptoms like weakness, depression, memory loss, weight gain, skin dryness, hoarseness, constipation, and cold intolerance.Physical examination, laboratory tests (thyroid function tests, including TSH, T3, and T4 levels), and imaging studies (such as thyroid ultrasound) may be necessary for diagnosis. Treatment often involves discontinuing the causative agent and may include levothyroxine therapy to supplement thyroid hormone levels.
