Code first the underlying physiological condition causing the dementia. Do not use this code if the dementia is due to Alzheimer's disease or vascular dementia.Specify the severity of the dementia using the appropriate 5th character (A, B, C). If behavioral disturbances are present, use F02.81.

Medical necessity is established by the diagnosis of dementia and the underlying physiological condition. Documentation should support the need for diagnostic testing and treatment.

Providers diagnose this condition based on patient history, signs, symptoms, physical and neuropsychological exams. Laboratory and imaging studies help identify underlying causes and rule out other conditions. Treatment addresses the underlying disease and may include medication and lifestyle changes.

IMPORTANT:If behavioral disturbances are present, use F02.81. For mild, moderate, or severe dementia, use F02.A0, F02.B0, or F02.C0, respectively. Consider codes F10-F19 with .17, .27, .97 for dementia in alcohol and psychoactive substance disorders and F01.5-, F01.A-, F01.B-, F01.C- for vascular dementia.

Example 1: A patient with Parkinson's disease develops dementia, but without behavioral disturbances. The underlying Parkinson's disease is coded first (G20), followed by F02.80., A patient with HIV develops dementia due to the infection, without behavioral disturbances. HIV (B20) is coded first, then F02.80., A patient diagnosed with hypothyroidism develops dementia.  The dementia is without behavioral disturbance.  Hypothyroidism (E03.-) is coded first followed by F02.80.

Documentation should include the type of dementia, the underlying physiological condition, the severity of the dementia, and the absence of behavioral disturbances. Supporting documentation such as neuropsychological evaluations, laboratory results, and imaging studies should also be included.
