2025 ICD-10-CM code G89

Pain, not elsewhere classified. This code also includes related psychological factors associated with pain.

Do not use G89 if a more specific code for the pain exists. Code also any related psychological factors associated with the pain (F45.42).

Medical necessity for G89 must be supported by documentation showing that the pain is significant enough to impact the patient's function and quality of life, and that other specific pain diagnoses have been ruled out.

Providers diagnose pain, not elsewhere classified, based on medical history, signs and symptoms, and physical examination. Laboratory studies (blood, urine, or CSF) may be used to diagnose underlying conditions. Imaging studies (X-rays, CT scan, MRI) and electrodiagnostic studies (EMG, NCV) may also be performed. Treatment depends on the cause, nature, duration, and severity of the pain and may include NSAIDs, muscle relaxants, anxiolytics, antidepressants, physical therapy, or biofeedback.

In simple words: This code represents pain that isn't classified elsewhere. It can be short-term (acute) or long-term (chronic) pain. It includes pain from nerve damage, injuries, surgery, and other causes, as long as there isn't a more specific code for it.It also includes any psychological factors related to the pain.

Pain, not elsewhere classified, refers to pain lasting for a few days to three months (acute) or long-lasting pain (chronic), not represented by another code. This category includes central pain syndrome (including Déjérine-Roussy, myelopathic pain, and thalamic pain syndromes), acute or chronic pain due to trauma, post-thoracotomy pain, other postprocedural pain, and other chronic pain.Code also any related psychological factors associated with the pain.

Example 1: A patient experiences chronic pain after a car accident, which is not localized to a specific area and does not fit any other pain diagnosis criteria. The pain has lasted for over a year and is associated with psychological distress. G89 is used., A patient has persistent burning pain in their foot, with no apparent underlying cause or injury. Nerve conduction studies are normal, but the pain significantly impacts their daily activities. G89 is used to code the pain., Following a thoracotomy, a patient experiences persistent pain in the chest and back that lasts for several months. This pain is not attributable to any other condition. G89 is used to code this post-procedural pain.

Documentation should clearly describe the characteristics of the pain (location, quality, severity, duration), associated symptoms, any diagnostic tests performed, and the impact on the patient's functionality. It should also document any psychological factors associated with the pain.

** Excludes1: generalized pain NOS (R52), pain disorders exclusively related to psychological factors (F45.41), pain NOS (R52).
