Medical necessity is established by the presence of signs and symptoms of infection, such as fever, pain, swelling, redness, or purulent drainage at the surgical site or within the affected organ/space.

IMPORTANT:For sepsis following a postprocedural wound infection related to an obstetrical procedure, sequence O86.03 first, followed by T81.44 (Sepsis following a procedure) or O86.04 (Sepsis following an obstetrical procedure), and an additional code identifying the infectious agent.

Documentation should clearly indicate the presence of infection, its location (organ or space), the relationship to the obstetric surgical procedure, and the type of surgical wound if applicable.If sepsis is present, document signs and symptoms supporting the diagnosis.
