Medical necessity is established by the presence of a superficial bite injury to the right eyelid and periocular area requiring medical evaluation and treatment.

Providers diagnose the condition based on patient history and physical examination. Treatment may include cleaning the area, applying ice packs, topical antihistamines and antibiotics, and oral pain relievers, anti-allergy medications, or NSAIDs.

In simple words: A minor bite on the surface of the right eyelid and the surrounding area. This is not a deep or serious bite and is the patient's first visit for this injury.

This is the first time the child has been seen for this injury., A patient comes to the emergency room after a cat scratched their right eyelid. The injury is superficial but has caused some irritation. This is their initial visit for the cat scratch.

Documentation should include the cause of the bite (animal or human), location and description of the injury (superficial, no deep tissue injury), and any associated symptoms (pain, redness, swelling).
