Poisoning by, adverse effect of, and underdosing of systemic antibiotics.

Code first the nature of the adverse effect if applicable. For poisoning or adverse effects, the drug giving rise to the adverse effect should be identified using codes from categories T36-T50 with a fifth or sixth character of '5'.

Medical necessity for the treatment of poisoning, adverse effects, or complications arising from antibiotic use should be clearly documented. The documentation must support the need for the level of care provided.

Clinicians are responsible for correctly identifying the causative agent, documenting the circumstances of the poisoning or adverse effect, and providing appropriate treatment and supportive care.

IMPORTANT:Use additional external cause codes from Chapter 20 to specify the cause of injury or poisoning. Use codes T36.0-T36.9 to specify the type of antibiotic involved.

Example 1: A patient intentionally overdoses on penicillin, requiring emergency medical treatment., A patient experiences severe gastrointestinal upset after taking a prescribed course of amoxicillin, requiring a change in medication., A patient mistakenly ingests their pet's antibiotic medication, leading to an adverse reaction and a visit to the emergency room.

Documentation should include the specific antibiotic involved, the route of administration, the dosage taken (if known), the time of ingestion or administration, the symptoms experienced, and any treatment provided.For adverse effects, document the nature of the reaction and its severity.

** Excludes1: antineoplastic antibiotics (T45.1-) locally applied antibiotic NEC (T49.0) topically used antibiotic for ear, nose and throat (T49.6) topically used antibiotic for eye (T49.5). For underdosing, use additional codes to specify manifestations of poisoning (e.g., underdosing during medical and surgical care (Y63.6, Y63.8-Y63.9), underdosing of medication regimen (Z91.12-, Z91.13-)).
