Poisoning by, adverse effect of and underdosing of cocaine.

Use secondary code(s) from Chapter 20 (External causes of morbidity) to indicate the cause of injury or poisoning, unless the circumstance and intent are documented as accidental. Use additional code(s) to specify manifestations of poisoning. For adverse effects, first code the nature of the adverse effect, and use T40.5 with a fifth or sixth character of 5 to identify cocaine as the causative agent. Use additional code to identify any retained foreign body, if applicable (Z18.-).

Modifiers may apply to indicate the episode of care or other specific circumstances.

Medical necessity must be established based on the specific scenario. For poisoning or adverse effects, the need for treatment is generally evident. For underdosing, the impact of the insufficient dosage on the patient's condition needs to be documented.
