Example 1: A patient presents to the emergency room after accidentally ingesting a cleaning solution. The specific ingredients are unknown, and the product label is unavailable. The patient exhibits symptoms of poisoning, including nausea, vomiting, and abdominal pain., A child is brought to the clinic after chewing on a plant in the garden. The plant is not identifiable. The child develops a rash and itching., A worker in a chemical factory is exposed to an unknown fume. The worker experiences difficulty breathing and dizziness.

The documentation should clearly state the circumstances of the exposure, the suspected substance if known, and the signs and symptoms exhibited by the patient. If the substance is unknown, this should be explicitly documented. The route of exposure (ingestion, inhalation, dermal contact, etc.) should also be noted. Any diagnostic tests performed and their results should be included.

** When no intent is indicated code to accidental. Undetermined intent is only for use when there is specific documentation in the record that the intent of the toxic effect cannot be determined. Use additional code(s) for all associated manifestations of toxic effect, such as: respiratory conditions due to external agents (J60-J70) and personal history of foreign body fully removed (Z87.821).
