2025 ICD-10-CM code V58.4

Person boarding or alighting a pick-up truck or van injured in a noncollision transport accident.

Use an additional code to identify the specific injury (e.g., a sprained ankle, fractured wrist). If the accident occurred on a public highway, appropriate codes from the Y92.4- series should be used. If airbag injury is involved, use W22.1-. If a cell phone or electronic device was in use at the time of the accident, use Y93.C-.Do not use V58.4 if the accident involved a heavy transport vehicle or if the injury was intentionally self-inflicted.

The 7th character of the code indicates the encounter type (A=initial encounter, D=subsequent encounter, S=sequela).

Medical necessity is established by the injury sustained due to the accident. The documentation must support the injury and the circumstances surrounding the incident.

The clinical responsibility is to document the nature of the injury sustained while boarding or alighting the vehicle, specify the location of the incident (if not a public highway), and rule out any collision involvement.

In simple words: This code is used when someone is hurt while getting into or out of a pick-up truck, van, SUV, or similar vehicle, but there was no crash.

This code describes an injury sustained while boarding or alighting (getting into or out of) a pick-up truck or van, where no collision occurred. This includes minivans, minbuses, sport utility vehicles (SUVs), trucks, and vans.It excludes injuries involving heavy transport vehicles (V60-V69).

Example 1: A patient twisted their ankle while stepping out of their SUV. There was no collision. , A patient fell while climbing into the back of a pick-up truck, sustaining a wrist fracture. The truck was stationary and there was no collision., A child hit their head while getting out of a minivan. The minivan was parked and there was no collision.

Documentation should clearly indicate the activity of boarding or alighting the vehicle, the type of vehicle, and that the injury occurred without a collision. The specific injury and location of the incident should also be documented.
