Failure in dosage during other surgical and medical care. This code is used to indicate a failure in the dosage of a medication or other substance during a surgical or medical procedure, excluding accidental overdoses or administering the wrong drug.

Use this code as a secondary code to the code describing the nature of the condition resulting from the dosage failure.

Medical necessity must be established by demonstrating that the dosage failure directly resulted in an adverse effect, additional treatment, prolonged hospitalization, or other significant clinical impact.

It is the clinician's responsibility to accurately document the specific type of dosage failure and any related complications.

Additional anesthetic is administered to ensure adequate sedation., A patient undergoing radiation therapy receives an overdose of radiation due to a malfunctioning machine. The incident is reported, and appropriate monitoring and treatment are provided for potential side effects.

Documentation should clearly specify the type of dosage failure, the substance involved, the administered dosage, the intended dosage, the method of administration, the time and date of the event, any corrective actions taken, and any resulting complications or adverse reactions.
