2025 CPT code 11770

Excision of pilonidal cyst or sinus; simple

This code is for a simple excision without extensive dissection or complications. If the excision involves multiple layers of closure, or if there is extensive dissection or complications, use 11771 or 11772, respectively. For incision and drainage only, use 10080 or 10081.

Modifiers may be applicable. Refer to current CPT coding guidelines for appropriate usage.

Medical necessity for this procedure is established when the pilonidal cyst or sinus causes pain, discomfort, recurrent infections, or drainage.

IMPORTANT:For extensive excision, use 11771. For complicated excision, use 11772. For incision and drainage, use 10080 (simple) or 10081 (complicated).

In simple words: Removal of a pilonidal cyst (a sac containing hair and skin debris) near the tailbone, which is a relatively straightforward procedure without any complications.

Surgical removal of a pilonidal cyst or sinus that does not require extensive dissection and is without complication. This includes simple single-layer closure of the wound.

Example 1: A patient presents with a small, non-infected pilonidal cyst near the tailbone. The cyst is surgically removed using a simple excision technique with single-layer closure., A patient has a recurring pilonidal cyst that is causing discomfort. The cyst is completely excised with a simple, single-layer closure., A patient presents with a pilonidal sinus (a hollow passage) that is not infected and does not involve deep or extensive tracts. The sinus is excised, and the wound is closed with a single layer of sutures.

Documentation should include the size and location of the cyst or sinus, the complexity of the excision, the type of closure used, and any associated complications.
