Immediate insertion of a breast implant following a mastectomy.

Follow current CPT coding guidelines and payer-specific guidelines.Code 19342 should not be reported together with 19328 (removal of intact implant) for the same breast.

Modifiers 50 (bilateral procedure) and others may be applicable depending on the specific circumstances of the procedure.

Medical necessity for code 19340 is established when immediate breast reconstruction is medically appropriate given the patient’s clinical condition. Factors considered can include the patient’s overall health and surgical risk, the type of mastectomy, and the patient's personal preferences.

The surgeon is responsible for creating the pocket for the implant, inserting the implant, and closing the incision.Anesthesiology services may also be involved.

IMPORTANT:For delayed insertion of a breast implant, use CPT code 19342.For removal of a tissue expander with concurrent insertion of a permanent breast implant, use CPT code 11970. If more extensive capsular revision is needed with code 11970, add code 19370. For removal of an intact breast implant without replacement, use code 19328. For removal of a ruptured breast implant, including implant contents, use code 19330. For a complete capsulectomy (removal of the implant and all intracapsular contents), use code 19371.For revision of a reconstructed breast, use code 19380.For autologous fat grafting for volume increase, use codes 15771 or 15772.For biologic implant for soft tissue reinforcement, use code 15777 in conjunction with the primary procedure.

Operative report detailing the mastectomy and implant placement, including implant type, location (subpectoral or subglandular), and any complications.Preoperative and postoperative imaging (if applicable). Pathology report confirming the diagnosis of cancer (if applicable). Consent form signed by the patient.
