2025 CPT code 20520

Removal of foreign body in muscle or tendon sheath; simple.

Do not report this code if the removal does not involve an incision. If the foreign body is subcutaneous, use codes 10120 or 10121 instead. For deeper or more complicated removals, use 20525.

Modifiers may be applicable, such as 51 (Multiple Procedures) if performed with other procedures, or 76 (Repeat Procedure or Service) if the removal requires multiple attempts.

Medical necessity is established by the presence of a foreign body causing symptoms such as pain, inflammation, infection risk, or limited mobility.

The physician prepares the patient, administers anesthesia, makes an incision, locates and removes the foreign body, cleans the area, and closes the incision. They may use imaging guidance.

IMPORTANT:For deeper or more complicated removals, use 20525. For subcutaneous foreign body removals, see 10120 and 10121.If no incision is needed, an E/M code may be appropriate.
