Injection(s) of single or multiple trigger point(s) into one or two muscles.

Code is reported per muscle(s) injected, not per injection. The number of injections does not affect code selection.

Modifiers may be applicable. For example, modifier 50 is used for bilateral procedures (injections into the same muscle on both sides of the body), Modifier 25 if a significant, separately identifiable E/M service is provided on the same day, Modifier 59 for a distinct procedural service.

Medical necessity should be supported by documentation of the patient's pain, functional limitations, and failed conservative treatments (if any).

The physician palpates the muscle to locate the trigger point, assesses for referred pain and a twitch response, and then injects the medication.

IMPORTANT:For injections into three or more muscles, use 20553.
