2025 CPT code 20661

Application of halo, including removal; cranial.

This code includes removal of the halo. Imaging guidance is reported separately. Modifier 51 should be used when halo application is performed in conjunction with other procedures.

Modifiers are applicable. Common modifiers include 22 (Increased Procedural Services), 51 (Multiple Procedures), and others as appropriate.

Medical necessity for cranial halo application is established by documenting the underlying condition requiring stabilization, such as a fracture, dislocation, instability, or post-surgical requirement.

The physician selects a correctly sized halo ring, positions it on the skull, and inserts pins. After securing the halo, the physician confirms proper cervical spine alignment using imaging.

In simple words: A cranial halo is a head device used to stabilize the neck after injury or surgery. It’s a ring attached to the skull with pins, and a vest is worn to provide support. The procedure includes putting on and taking off the halo.

Application of a cranial halo device, including removal. This involves attaching a ring-shaped device to the skull with pins to provide traction and stabilize the cervical spine. The procedure includes the removal of the halo.

Example 1: A patient with a cervical fracture requires stabilization. A cranial halo is applied to immobilize the neck and allow for healing., Following cervical spine surgery, a cranial halo is applied to maintain stability and protect the surgical site., A patient with cervical instability due to a medical condition requires a cranial halo for long-term stabilization.

Documentation should include the reason for halo application, type of halo used, details of application and removal, and confirmation of cervical spine alignment.
