Bone marrow aspiration for bone grafting during spine surgery, through a separate incision.

This code should only be reported when bone marrow aspiration is performed through a separate incision during spine surgery and used for bone grafting.It is reported separately from the primary spinal procedure code. For bilateral procedures, report 20939 twice. Modifier 62 should not be used with 20939.

Modifiers, such as 59 (distinct procedural service), 76 (repeat procedure), 77 (repeat procedure by another physician), 78 (return to OR), 79 (unrelated procedure), 80 (assistant surgeon), 81, 82, AS, LT, and RT are applicable where appropriate.

Medical necessity for 20939 is established by the need for bone grafting during the spine surgery. The documentation must support the medical reason for both the spinal procedure and the bone graft.

The physician performs the bone marrow aspiration during the same operative session as the spine surgery, but through a separate incision. This involves preparing the aspiration site, making the incision, inserting the needle, aspirating the marrow, controlling any bleeding, and then using the harvested marrow during the primary spinal procedure.

IMPORTANT:For bone marrow aspiration for other therapeutic musculoskeletal applications (not spine surgery), use 20999. For diagnostic bone marrow aspirations, use 38220 or 38222. For bone marrow aspirations for platelet-rich stem cell injections, use 0232T.
