2025 CPT code 21235

Autogenous ear cartilage graft to nose or ear (includes obtaining graft).

Refer to the official CPT coding guidelines for comprehensive instructions on appropriate code usage.Always consider the specific details of the procedure performed before selecting the most accurate and appropriate code.

Modifiers may be applicable in certain circumstances (e.g., modifier 59 for distinct procedural service). Consult current CPT guidelines for applicable modifiers.

The procedure must be medically necessary to correct a structural deformity, not for cosmetic reasons.Documentation supporting the medical necessity should be included. Preauthorization may be needed for certain payers.

The surgeon is responsible for all aspects of the procedure, including harvesting the cartilage graft, preparing the recipient site, implanting the graft, and closing incisions.A subsequent rhinoplasty may be required if the graft is placed on the nose. Postoperative care and follow-up should be documented separately.

IMPORTANT For graft augmentation of facial bones, use 21208.Do not report this code if harvesting ear cartilage in association with reconstructive procedure that includes graft harvesting (eg, tympanoplasty, myringoplasty), unless a prior surgical procedure requires obtaining graft from the opposite ear due to lack of cartilage. In this exception, append modifier 59.

In simple words: The doctor takes cartilage from the patient's ear and uses it to repair a deformity on the nose or ear caused by a fracture, injury, tumor removal, or a birth defect. This includes the removal of the cartilage, preparation of the area for the graft, placing the graft, and closing the incisions.

This CPT code encompasses the surgical procedure involving harvesting autogenous cartilage from the patient's ear and grafting it to the nose or ear.The procedure includes the procurement of the graft, preparation of the recipient site, placement of the graft, and closure of incisions. If the graft is to the nose, a subsequent rhinoplasty may be performed to achieve optimal cosmetic results.Appreciable vessel exploration and/or neuroplasty should be billed separately.

Example 1: A patient presents with a nasal deformity following a previous fracture. The surgeon performs a harvest of autogenous ear cartilage and subsequent grafting to reconstruct the nose., A patient has a congenital ear deformity. The surgeon uses autogenous ear cartilage from the opposite ear to reconstruct the patient's deformed ear., A patient presents with a post-surgical ear defect following tumor resection. The surgeon performs an autogenous ear cartilage graft to repair the defect.

* Preoperative diagnosis and justification for the procedure.* Detailed surgical notes including type of graft, size, location of harvest and implantation sites.* Intraoperative photos or images.* Postoperative progress notes, including any complications.* Indication of medical necessity (if applicable).

** This code should not be reported with codes for procedures that already include harvesting of ear cartilage, unless obtaining graft from opposite ear due to lack of cartilage from one ear.Always ensure complete and accurate documentation for proper reimbursement.
