2025 CPT code 23075

Excision of a subcutaneous soft tissue tumor in the shoulder area, measuring less than 3 cm.

Refer to CPT guidelines for soft tissue tumor excisions. Do not report this code if the tumor is larger than 3 cm or if a radical resection is performed.Do not separately report the application and removal of the first cast, splint, or traction device.

Modifiers may be applicable to indicate specific circumstances, such as bilateral procedures (modifier 50) or reduced services (modifier 52).

Medical necessity for this procedure is established by the presence of a symptomatic or suspicious soft tissue tumor in the shoulder area.Documentation should support the need for surgical excision, such as pain, functional impairment, or concern for malignancy.

The physician prepares the patient, administers anesthesia, makes an incision, excises the tumor and a margin of surrounding tissue, sends the specimen for analysis, irrigates the wound, and closes the incision.

In simple words: Removal of a small lump (less than 3 cm) located just under the skin in the shoulder area.

Excision, tumor, soft tissue of shoulder area, subcutaneous; less than 3 cm. This code describes the surgical removal of a tumor located in the subcutaneous tissue (beneath the skin but above the deep fascia) of the shoulder area. The tumor's largest diameter, including margins, is less than 3 cm. The procedure includes simple or intermediate repair of the wound.

Example 1: A 25-year-old patient presents with a palpable, asymptomatic lipoma measuring 2 cm in the subcutaneous tissue of the left shoulder. The lipoma is excised under local anesthesia using a marginal excision technique., A 45-year-old patient has a painful 1.5 cm subcutaneous neurofibroma on the right shoulder. The neurofibroma and a margin of surrounding tissue are excised, and the wound is closed with sutures., A 60-year-old patient with a history of radiation therapy presents with a 2.8 cm subcutaneous malignant fibrous histiocytoma on the posterior aspect of the shoulder. The tumor is excised with a small margin of healthy tissue.

Documentation should include the size and location of the tumor, the type of excision performed (e.g., simple, marginal), and the method of wound closure. A pathology report should be obtained to confirm the diagnosis.
