Modifiers 22, 47, 50-53, 54-59, 62, 73, 74, 76-82, 99, and others as applicable can be used.
Medical necessity should be supported by documentation of failed conservative treatment, functional limitations, pain, or deformity caused by the nonunion or malunion. Radiographic evidence of nonunion/malunion should also be provided.
