2025 CPT code 27226

Open treatment of posterior or anterior acetabular wall fracture, with internal fixation.

Use this code only when internal fixation is performed.If other procedures like bone grafting are done through separate incisions, they may be reported separately. If the surgeon removes and replaces the cast, code it separately. Do not use modifier 56 as casting is not considered pre-operative care.

Modifiers may be applicable to 27226. For instance, modifier 54 (Surgical Care Only) may be used if the surgeon only performs the surgical portion of the procedure, while modifier 76 (Repeat Procedure by Same Physician) would indicate a re-reduction of the fracture.

Medical necessity must be established for CPT 27226.The documentation should clearly demonstrate the need for surgical intervention due to instability or displacement of the acetabular fracture that cannot be managed conservatively.
