Treatment of intertrochanteric, peritrochanteric, or subtrochanteric femoral fracture; with intramedullary implant, with or without interlocking screws and/or cerclage.
Modifiers may be applicable in specific situations (e.g., 76 for repeat procedure, 54 for surgical care only).
Medical necessity is established by the presence of a femoral fracture requiring surgical intervention for stability and proper healing.
The physician is responsible for the entire surgical process, including prepping the patient, anesthesia, incision, fracture reduction, implant placement, wound closure, and post-operative care.
In simple words: This procedure fixes a broken thigh bone (femur) near the hip. The surgeon makes an incision and puts a metal rod inside the bone to stabilize it.Screws and/or a metal band may also be used to hold the broken pieces together. The skin is then stitched up, and the leg is put in a brace or splint for several weeks.
This procedure involves open treatment of a femoral fracture between, around, or below the trochanters. The fracture is fixed using an intramedullary implant placed inside the canal of the femur to stabilize the fractured bones. Interlocking screws and/or cerclage (a metal band or wire loop wrapped around the fragments) may also be used to hold the fracture fragments in place. The surgeon incises the skin over the upper end of the femur, dissects through subcutaneous tissue, and retracts or detaches muscles to expose the fracture. The bone is then adjusted to reduce the fracture fragments. The intramedullary nail (IMN) is inserted through the upper fractured fragment and passed through the medullary canal into the femoral shaft. Screws may be drilled through the nail into the femoral head, and cerclage may be used for added stability. The wound is closed by reattaching muscles and suturing the tissue layers. The leg is placed in a brace or splint for about four to six weeks. An X-ray may be performed to confirm fracture reduction.
Example 1: A patient sustains a peritrochanteric femoral fracture in a fall and requires open reduction and internal fixation with an intramedullary nail., A patient with an intertrochanteric femoral fracture undergoes surgery for stabilization using an intramedullary implant and interlocking screws., A patient with a subtrochanteric femoral fracture requires open treatment with an intramedullary nail and cerclage due to comminution (multiple fracture fragments).
Documentation should include details of the fracture, type of implant used, operative report, and post-operative care plan.
** For an intertrochanteric, peritrochanteric, or subtrochanteric femoral fracture treated with a plate and screw type implant, use code 27244.
