2025 CPT code 27558

Open treatment of knee dislocation, with internal fixation and primary ligamentous repair, including augmentation/reconstruction

Use this code when open treatment of a knee dislocation is performed with primary ligament repair/reconstruction and may or may not include internal or external fixation.Do not report separate codes for the internal/external fixation or ligament repair/reconstruction.

Modifiers may be applicable. Common modifiers include 22 (increased procedural services), 50 (bilateral procedure), 54 (surgical care only), 76 (repeat procedure by same physician), 77 (repeat procedure by another physician), and others as appropriate.

Medical necessity for 27558 is established by the presence of a knee dislocation requiring surgical intervention for reduction and stabilization due to instability and/or associated ligamentous damage that cannot be managed with closed treatment.

The physician is responsible for performing the open reduction and internal fixation of the dislocated knee, including the repair or reconstruction of the primary ligament. This includes pre-operative planning, surgical execution, and post-operative care.

In simple words: This procedure involves surgically repairing a dislocated knee. The surgeon makes an incision to access the knee joint, puts the bones back into their proper position, and uses hardware like pins or screws to stabilize the joint. The surgeon also repairs the main ligaments in the knee and may strengthen or rebuild them using a graft.

Open treatment of knee dislocation, with or without internal or external fixation; with primary ligamentous repair or reconstruction.

Example 1: A patient dislocates their knee during a sporting event, tearing the anterior cruciate ligament (ACL). The surgeon performs an open reduction to relocate the knee joint, uses screws to stabilize it, and reconstructs the ACL using a graft., A patient falls and dislocates their knee, damaging the posterior cruciate ligament (PCL). The surgeon performs an open reduction, internally fixates the knee, and repairs the PCL., A patient experiences a traumatic knee dislocation in a car accident, resulting in multiple ligament tears. The surgeon performs an open reduction, stabilizes the joint with internal fixation, and reconstructs the damaged ligaments.

Documentation should include details of the dislocation, associated injuries (ligament tears, fractures), type of internal or external fixation used, type of ligament repair/reconstruction, and any complications encountered.Pre- and post-operative diagnoses, operative report, and progress notes are also required.
