The medical necessity for additional tendon transfers must be documented in the patient's chart. This often includes the severity and impact of the additional tendon injury on the patient's function and the surgeon's justification for the additional procedure.

The surgeon is responsible for pre-operative planning, incision, exposure of the diseased tendon, excision of the diseased portion, harvesting of the healthy tendon, preparation and rerouting of the tendon graft, placement and fixation of the tendon graft, hemostasis, wound closure, and post-operative care.

IMPORTANT This code is always used with either 27690 or 27691.

In simple words: This code is for additional tendon repairs or replacements done during the same surgery.If a doctor fixes one tendon and then fixes one or more others at the same time, this code is used to bill for the extra tendons.

This code reports the transfer or transplant of one or more additional tendons during the same operative session as a primary tendon transfer or transplant (27690 or 27691).It is an add-on code and should only be reported in conjunction with a primary procedure code (27690 or 27691). The procedure involves harvesting a healthy tendon, preparing it, and rerouting it to the required location, followed by fixation and closure.The number of additional tendons transferred determines the number of units of 27692 reported.

Example 1: A patient presents with chronic tendon rupture in the posterior tibial tendon and peroneus longus tendon.The surgeon performs a primary tendon transplant of the posterior tibial tendon (27690) and subsequently repairs the peroneus longus tendon using a transfer from a flexor digitorum longus tendon. 27692 is added., A patient sustains multiple tendon injuries to the foot after an accident. During the surgical repair, the surgeon first transfers the flexor hallucis longus tendon (27691) to replace a damaged peroneus brevis tendon.Additionally, the surgeon repairs two additional damaged tendons.Two units of 27692 are reported., During a foot reconstruction surgery for a patient with a dropped foot, the primary procedure involves the transplantation of the tibialis posterior tendon (27690).An additional peroneus brevis tendon is also transferred during the same operative session. One unit of 27692 is added.

Detailed operative notes describing the need for the additional tendon transfer(s), the tendons involved, the technique used for the transfer, and the location of the graft.Preoperative and postoperative imaging should be included.Patient’s symptoms and functional status before and after the procedure must be documented.
