2025 CPT code 29580

Application of an Unna boot.

Refer to the CPT guidelines for surgical procedures on the musculoskeletal system.Appropriate modifiers (e.g., 50 for bilateral procedures, RT/LT for laterality) should be appended as needed.

Modifiers 50 (Bilateral Procedure), RT (Right side), LT (Left side) may be applicable depending on the clinical scenario.

Medical necessity for 29580 is established when a patient presents with a condition that requires compression therapy, such as venous stasis ulcers, sprains, or strains. The Unna boot provides a controlled compression environment to promote healing and reduce edema.

The provider is responsible for proper application and removal of the Unna boot, ensuring proper fit and pressure. They assess the patient's condition and determine the need for subsequent treatments.

IMPORTANT:Do not report 29580 in conjunction with 29540, 29581 for the same extremity.

In simple words: The doctor wraps your leg with a special bandage called an Unna boot to help heal leg sores or injuries. This bandage keeps the skin moist and applies pressure to improve blood flow and healing.

Application of a Unna boot, a spiral compression dressing made of cotton and zinc oxide paste or other emollients, to treat venous stasis ulcers by controlling blood flow and providing a stable environment for ulcer healing. Also used for strains and sprains. The procedure includes placement of the foot in neutral position, cleaning and drying the area, applying a moisture barrier cream (avoiding the ulcerated area), applying a foam pad to an actively draining ulcer (if present), wrapping the leg with the bandage from toes to below the knee, overlapping layers, and securing with a self-adherent wrap.Assessment of fit and pressure is included. Removal and/or replacement after 3-7 days is also included.

Example 1: A patient presents with a venous stasis ulcer on their lower leg. The physician applies an Unna boot to promote healing by managing edema and providing a moist environment., An athlete sustains a moderate ankle sprain. The physician applies an Unna boot to provide compression and support, promoting stability and reducing inflammation., A patient with chronic venous insufficiency experiences recurrent leg swelling. The physician utilizes an Unna boot as part of a comprehensive treatment plan to manage edema and prevent ulcer formation. The boot is changed every 3-7 days as needed.

* Patient's medical history, including any relevant allergies.* Detailed assessment of the affected extremity, including measurements and photographic documentation of the wound (if applicable).* Documentation of the Unna boot application technique, including size, layers, and type of dressing materials used.* Note of any complications, such as skin irritation or allergic reactions.* Date and time of application and removal of the Unna boot.* Follow-up plan and patient education regarding wound care.
