2025 CPT code 31571

Laryngoscopy, direct, with injection into vocal cord(s), therapeutic; with operating microscope or telescope.

For endoscopic procedures, report appropriate endoscopy of each anatomic site examined. Laryngoscopy includes examination of the tongue base, larynx, and hypopharynx.

Modifiers may be applicable. Refer to the provided list of modifiers for appropriate usage.

Medical necessity must be established by documenting the patient's condition and symptoms, such as dysphonia, airway obstruction, or aspiration, and how the procedure will address the issue.

The physician prepares the patient, administers anesthesia, inserts the laryngoscope, injects the therapeutic agent into the vocal cord(s) using an operating microscope or telescope, and assesses the outcome.

IMPORTANT:Do not report 31571 in conjunction with 69990.

In simple words: The doctor uses a special instrument to look into your voice box and inject medicine into your vocal cords to help them work better.

This code describes a procedure where the physician performs a direct laryngoscopy using an operating microscope or telescope to visualize the larynx and surrounding structures. Then, they inject a therapeutic agent into one or more vocal cords. This is often done to treat vocal cord paralysis.

Example 1: A patient with unilateral vocal cord paralysis undergoes direct laryngoscopy with injection of a bulking agent into the affected vocal cord to improve voice quality., A patient with bilateral vocal cord paralysis undergoes injection augmentation to improve airway patency., A patient with vocal cord scarring receives an injection of a therapeutic agent to improve vocal cord mobility and reduce dysphonia.

Documentation should include the reason for the procedure, such as vocal cord paralysis or scarring. The type of therapeutic agent injected, the location of the injection, and the use of the operating microscope or telescope should also be documented. Additionally, any pre- and post-procedure assessments of vocal cord function should be noted.
