2025 CPT code 33305

Repair of cardiac wound with cardiopulmonary bypass.

Use 33305 when the repair of a cardiac wound is performed with cardiopulmonary bypass.Do not report separately for the cardiopulmonary bypass itself, as it is included in the code.

Modifiers may be applicable. Refer to current CPT guidelines.

Medical necessity must be established by documenting the specific cardiac injury requiring surgical repair. This typically involves a life-threatening condition or significant impairment of cardiac function.

The surgeon performs the procedure, including opening the chest, establishing bypass, repairing the wound, and closing the incision.

IMPORTANT Use 33300 for repair of cardiac wound without cardiopulmonary bypass. For cardiac assist services, use codes 33946, 33947, 33948, 33949, 33967-33983, 33990, 33991, 33992, 33993, 33995, and 33997.

In simple words: The doctor fixes a cut or injury to the heart while the patient is connected to a heart-lung machine. This machine keeps the blood flowing and adds oxygen to it while the heart is being repaired.

This procedure involves repairing a wound to the heart while the patient is on cardiopulmonary bypass (CPB).A thoracotomy or sternotomy is performed to access the heart, and a rib spreader is used.CPB is established, and the pericardial sac is opened. Clotted blood and fluid are removed, and hemostasis is achieved. The heart is inspected for all wounds, which are then repaired with sutures, possibly over a pad or synthetic graft. The patient is weaned off bypass, incisions are closed, and drainage tubes may be inserted.

Example 1: A patient with a stab wound to the heart requiring immediate surgical repair., A patient undergoing open-heart surgery who experiences an accidental tear in the heart muscle., A patient with a gunshot wound to the heart.

Documentation should include operative report detailing the location and extent of the cardiac wound, the use of cardiopulmonary bypass, the method of repair, and any complications. Any pre- or post-operative diagnoses, such as cardiac tamponade, should also be documented.
