Surgical ventricular restoration procedure, including prosthetic patch.
Modifiers may be applicable. Modifier 22 may be used if the service is substantially more complex than typically required. Modifier 80 is used if an assistant surgeon was required.
Medical necessity is established by documentation of left ventricular dysfunction and enlargement caused by scar tissue following myocardial infarction or other cardiac conditions, and symptoms of heart failure not adequately managed by medical therapy.
The surgeon performs the procedure with the patient under general anesthesia and using cardiopulmonary bypass. A median sternotomy incision is made to access the heart. The damaged portion of the left ventricle is identified and reshaped using sutures and a prosthetic patch to create a more normal size and shape. The patient is then weaned off bypass, and the incision is closed.

Example 1: A patient with a history of myocardial infarction and a significantly enlarged and dysfunctional left ventricle undergoes surgical ventricular restoration to improve heart function and reduce symptoms of heart failure., A patient with an aneurysm in the left ventricle undergoes this procedure to remove the aneurysm and reshape the ventricle using a patch, preventing potential rupture., Following a heart attack, a patient develops a ventricular septal defect. This procedure is performed to repair the defect and simultaneously reshape the left ventricle due to the damage from the infarction.
Documentation should include operative report detailing the procedure including size and location of the ventricular defect, type of prosthetic patch used, use of cardiopulmonary bypass, any concomitant procedures performed. Pre- and post-operative echocardiograms should be documented showing the improvement in left ventricular function and size.
