Surgical creation of a shunt from the subclavian artery to the pulmonary artery, a procedure known as a Blalock-Taussig type operation.
Modifiers may be applicable in certain situations, such as increased procedural services (modifier 22), multiple procedures (modifier 51), or assistant surgeon (modifier 80).
Medical necessity is established by the presence of a congenital heart defect that restricts pulmonary blood flow and causes cyanosis. The Blalock-Taussig shunt is medically necessary as a palliative measure to improve oxygenation until corrective surgery can be performed.
The surgeon is responsible for all aspects of the procedure, including prepping the patient, making the incision, creating the shunt, controlling bleeding, and closing the incision.
