2025 CPT code 34718
(Active) Effective Date: N/A Surgery - Cardiovascular System Feed
Endovascular repair of iliac artery using branched endograft, excluding concurrent aorto-iliac endograft placement.
Modifiers applicable: 50 (Bilateral Procedure)
In simple words: A minimally invasive procedure to repair a diseased iliac artery (the main artery in your pelvis) using a special Y-shaped graft. This procedure is used for issues like aneurysms, but not ruptures, and is done when a repair to the larger abdominal aorta isn't needed at the same time.
Endovascular repair of iliac artery, not associated with placement of an aorto-iliac artery endograft at the same session, by deployment of an iliac branched endograft, including pre-procedure sizing and device selection, all ipsilateral selective iliac artery catheterization(s), all associated radiological supervision and interpretation, and all endograft extension(s) proximally to the aortic bifurcation and distally in the internal iliac, external iliac, and common femoral artery(ies), and treatment zone angioplasty/stenting, when performed, for other than rupture (eg, for aneurysm, pseudoaneurysm, dissection, arteriovenous malformation, penetrating ulcer), unilateral. (For bilateral placement of an iliac branched endograft, report modifier 50) (Do not report 34718 in conjunction with 34701, 34702, 34703, 34704, 34705, 34706, 34707, 34708, 34709, 34717) (Do not report 34718 in conjunction with 34710, 34711 on the same side) (For placement of an iliac branched endograft in the same setting as aorto-iliac endograft placement, use 34717) (For placement of an isolated iliac branched endograft for rupture, use 37799)
Example 1: A patient presents with an aneurysm in the right iliac artery. An iliac branched endograft is deployed to treat the aneurysm, excluding the aorta., A patient has a pseudoaneurysm in the left common iliac artery.Code 34718 is used when an iliac branched endograft is placed to exclude the pseudoaneurysm., A patient with an arteriovenous malformation in the iliac artery undergoes endovascular repair with a branched endograft. As this is an isolated iliac procedure without aortic involvement, code 34718 is reported.
Documentation should include details of the diagnosis, pre-procedure sizing and device selection, selective catheterizations performed, type of endograft used, any extensions used, angioplasty/stenting performed (if any), radiological supervision and interpretation findings, and confirmation of treatment zone limited to the iliac artery(ies).Specifically note that the procedure does *not* include concurrent repair of the abdominal aorta.
** Code 34718 represents a significant advancement in treating iliac artery diseases, offering a minimally invasive approach compared to open surgery.
