Modifiers may be applicable depending on the circumstances of the procedure. For example, modifier 51 (multiple procedures) might be used if other procedures are performed during the same session.

Medical necessity for 36425 is established when venous access is needed for blood collection or medication administration, and standard venipuncture techniques are not feasible (e.g., difficult vein access, urgent need for intravenous fluids).

The physician or qualified healthcare professional is responsible for performing the venipuncture, including site preparation, insertion of the needle or catheter, collection of the sample or injection of medication, and application of pressure to the site to control bleeding.

IMPORTANT:Do not report 36425 in conjunction with 36475, 36476, 36478. For patients under one year of age, see code 36420. For collection of a specimen from an established catheter, use 36592. For collection of a specimen from a completely implantable venous access device, use 36591.

A needle is carefully inserted into the vein, avoiding puncturing the posterior wall. Approximately 5 mL of blood is collected into a tube, or medication is introduced.The needle is then removed, and pressure is applied to the puncture site to stop any bleeding.

Example 1: A 25-year-old patient requires blood work for pre-operative testing. The physician performs a venipuncture to obtain a blood sample., A 60-year-old patient with peripheral intravenous access difficulties requires intravenous antibiotics. The physician performs a cutdown to establish intravenous access., A 12-year-old patient requires an emergency blood transfusion. A cutdown is performed to gain rapid intravenous access.

Documentation should include the patient's age, the reason for the venipuncture (e.g., blood draw, medication administration), the location of the venipuncture site, the volume of blood drawn or medication administered, and any complications encountered.
