2025 CPT code 36906
(Active) Effective Date: N/A Surgery - Dialysis Circuit Interventions Feed
Percutaneous transluminal mechanical thrombectomy and/or infusion for thrombolysis in the dialysis circuit, including imaging, radiological supervision and interpretation, diagnostic angiography, and intraprocedural pharmacological thrombolytic injections; with transcatheter placement of intravascular stent(s) in the peripheral dialysis segment, and all angioplasty within the peripheral dialysis circuit.
Modifiers may be applicable. Refer to current CPT guidelines for appropriate usage.
Medical necessity must be established by documenting the patient's clinical condition, the impact on dialysis access function, and the rationale for the chosen intervention.
In simple words: This procedure clears blood clots and opens narrowed blood vessels in the arm or leg used for dialysis. A tiny tube (catheter) is inserted into the dialysis access point, and a clot-busting drug may be injected. If there's a blockage, the doctor will inflate a tiny balloon to widen the vessel and may insert a small mesh tube (stent) to keep it open. X-ray imaging is used throughout the procedure.
This code encompasses a comprehensive procedure for managing thrombotic complications within a dialysis circuit. It involves the mechanical removal and/or infusion of thrombolytic agents to dissolve clots. This includes all necessary imaging, radiological supervision, and interpretation, diagnostic angiography, fluoroscopic guidance, catheter placements, and intraprocedural pharmacological thrombolytic injections.Additionally, the code includes the placement of a stent in the peripheral segment of the dialysis circuit, along with any necessary angioplasty in that segment, to maintain vessel patency. This procedure is performed percutaneously, meaning through the skin, minimizing invasiveness. All imaging and radiological supervision required for stent placement are also included.
Example 1: A patient with a thrombosed arteriovenous fistula (AVF) undergoes thrombectomy, angioplasty, and stenting of the peripheral dialysis segment., A patient with an occluded arteriovenous graft (AVG) requires thrombolysis, balloon angioplasty, and stent placement in the peripheral segment., A patient with a failing AVF presents with stenosis and intraluminal thrombus. The physician performs thrombectomy, followed by angioplasty and stenting to restore patency.
Documentation should include details of the procedure, including the location and extent of the thrombus/stenosis, the methods used for thrombectomy/thrombolysis, the type and size of the stent placed, the location of angioplasty, and any complications encountered.Pre- and post-procedure angiography findings should also be documented.
