2025 CPT code 37220

Revascularization, endovascular, open or percutaneous, iliac artery, unilateral, initial vessel; with transluminal angioplasty.

Refer to CPT guidelines for selective vascular catheterizations.This code includes introduction and all lesser order selective catheterizations used in the approach.Additional catheterizations within the same family of arteries should be coded with 36218 or 36248.Additional first order or higher catheterizations in different vascular families should be coded separately.

Modifiers applicable include 22 (Increased Procedural Services), 50 (Bilateral Procedure), 52 (Reduced Services), 53 (Discontinued Procedure), etc. as appropriate.

Medical necessity should be supported by documentation of the symptoms (e.g., claudication, rest pain) and diagnostic findings demonstrating significant stenosis or occlusion affecting blood flow to the limb.

The physician prepares and anesthetizes the patient, accesses the artery, inserts the guidewire and catheter, uses imaging guidance, inflates the balloon to expand the vessel, and closes the access site, monitoring the patient's vital signs throughout.

IMPORTANT:+37222 for each additional ipsilateral iliac artery treated with angioplasty. +37223 and 37221 for stent placement with or without angioplasty in additional or initial vessels, respectively.
