2025 CPT code 37222
(Active) Effective Date: N/A Surgery - Surgical Procedures on the Cardiovascular System Surgery Feed
Revascularization, endovascular, open or percutaneous, iliac artery, each additional ipsilateral iliac vessel; with transluminal angioplasty.
Modifiers may be applicable to further specify the circumstances of the procedure (e.g., laterality, bilateral procedures). Refer to current CPT coding guidelines for appropriate modifier usage.
Medical necessity must be supported by documentation demonstrating the presence of significant stenosis or occlusion in the iliac arteries causing symptoms or limiting blood flow, justifying the need for revascularization. The medical record should clearly link the procedure to the patient's condition and expected benefit.
The physician performs the procedure after the patient is prepped and anesthetized. This involves making a small incision or puncture, introducing a guidewire and catheter, using angiography for guidance, inflating the balloon to expand the artery, withdrawing the catheter, and suturing the incision/puncture site.Post-procedure vital signs and stability are monitored.
In simple words: This procedure opens up additional blocked arteries in the pelvis/leg area on the same side as the first artery treated during the same procedure. A small balloon is inserted into the artery and inflated to widen the blocked area.
This add-on procedure describes restoring blood supply in an additional narrowed or blocked ipsilateral iliac artery through an endovascular, open, or percutaneous approach with transluminal angioplasty. It involves placing a balloon catheter at the blockage site and inflating it to expand the vessel's lumen. This code is used for each additional ipsilateral iliac vessel treated after the initial vessel on the same side.
Example 1: A patient presents with multiple blockages in their right iliac arteries.37220 is used for the first artery treated, and 37222 is used for each additional artery treated on the right side., During an endovascular procedure, two blockages in the left iliac artery are identified. 37220 is reported for the initial iliac artery treated, and 37222 is reported for the second iliac artery treated., A patient has a blockage in both the left and right iliac arteries. 37220 and 37221 are reported for the initial treatment of arteries on both sides, respectively. During the same encounter, an additional stenosis on the right side requires treatment and is coded using 37222.
Documentation should include details of the initial and subsequent angioplasties, including the location of the blockages, the approach used (endovascular, open, or percutaneous), and the size of the balloon catheter.Angiography reports should be included. The medical necessity for treating each vessel should be clearly documented.
