2025 CPT code 37230

Revascularization, endovascular, open or percutaneous, tibial or peroneal artery, unilateral, initial vessel; with transluminal stent placement(s), includes angioplasty within the same vessel, when performed.

This code is for the initial vessel treated. Additional vessels treated should be reported with add-on codes.

Modifiers may be applicable, such as 50 for bilateral procedures, 52 for reduced services, 59 for distinct procedural service, 76, 77, and 78 for repeat procedures, LT or RT to indicate laterality.

Medical necessity must be established by documenting the patient's symptoms, clinical findings, and the severity of the stenosis, which justifies the need for revascularization.

The physician accesses the artery through an open or percutaneous approach. A catheter is advanced to the blockage, and a stent is deployed to open the narrowed area. Angioplasty within the same vessel may also be performed. The incision is repaired, or the puncture site is bandaged.

IMPORTANT:+37232 for each additional tibial/peroneal artery treated with angioplasty, +37233 for each additional unilateral tibial/peroneal artery treated with atherectomy, +37234 for each additional unilateral tibial/peroneal artery treated with stent placement, +37235 for each additional unilateral tibial/peroneal artery treated with atherectomy and stent placement(s).Use 37228 for initial vessel angioplasty, 37229 for atherectomy of initial vessel, and 37231 for atherectomy and stent placement of initial vessel.

In simple words: This procedure opens a blocked artery in the leg (tibial or peroneal artery) using a small mesh tube called a stent. The doctor may access the artery through a small incision or by inserting a catheter through the skin. This procedure restores blood flow to the leg and foot. The code also includes widening the narrowed artery with a balloon (angioplasty).

This procedure involves restoring blood supply to a narrowed tibial or peroneal artery using an endovascular, open, or percutaneous approach. It includes placing a stent within the vessel to open the blockage and includes angioplasty within the same vessel if performed. This code applies to the initial vessel treated.

Example 1: A patient presents with severe claudication in the right leg due to a stenosis in the right tibial artery. The physician performs an endovascular procedure to place a stent in the artery, restoring blood flow., A patient with critical limb ischemia has a blockage in the left peroneal artery. The physician performs an open procedure to access the artery and place a stent, followed by angioplasty., A patient with a severely narrowed tibial artery undergoes a percutaneous stent placement to improve blood flow and prevent amputation.

Documentation should include the diagnosis, the approach used (endovascular, open, or percutaneous), the location of the stenosis (tibial or peroneal artery), confirmation of stent placement, any additional procedures performed (e.g., angioplasty), and any complications.
