2025 CPT code 37235

Revascularization, endovascular, open or percutaneous, tibial/peroneal artery, unilateral, each additional vessel; with transluminal stent placement(s) and atherectomy, includes angioplasty within the same vessel, when performed.

Code 37235 is an add-on code and should not be reported alone. It must be used in conjunction with a primary code such as 37231.

Modifiers may be applicable to further specify the circumstances of the procedure. Consult modifier guidelines for appropriate usage.

Medical necessity must be established for each additional vessel treated and documented in the patient's medical record. This includes the severity of the stenosis, impact on blood flow, and symptoms. Supporting documentation like imaging results and clinical findings should justify the intervention.

The physician restores blood supply to additional blocked arteries in the lower leg using stents and plaque removal techniques.Approaches can be open, percutaneous, or endovascular.

IMPORTANT:(Use 37235 in conjunction with 37231)

In simple words: This additional procedure restores blood flow in each extra blocked artery in the lower leg (tibial or peroneal) on one side. It involves inserting and expanding a stent to open the blockage and removing plaque buildup. This procedure may also include widening the artery with a balloon.

This add-on procedure describes restoring blood supply in each additional stenosed, or narrowed, unilateral tibial or peroneal artery through an endovascular, open, or percutaneous approach. This procedure involves transluminal stent placement, where a stent is placed and expanded at the occlusion site, and atherectomy to remove plaque. Angioplasty within the same vessel is included when performed.

Example 1: A patient presents with multiple blockages in the tibial and peroneal arteries of the left leg. Code 37231 is used for the initial vessel treated, and 37235 is used for each additional vessel treated during the same procedure., A patient has a blockage in the right peroneal artery and two blockages in the right tibial artery.37231 would be reported once, and 37235 would be reported twice., A patient with severe peripheral artery disease has stenosis in multiple tibial and peroneal arteries in one leg.Following the initial treatment (37231), each subsequent artery treated with stent placement and atherectomy would be coded as 37235.

Documentation should include details of the approach (endovascular, open, or percutaneous), the vessels treated, the type of stent used, the atherectomy performed, and any associated angioplasty.Medical necessity for treating each additional vessel should also be documented.

** This code represents treatment of additional vessels during the same operative session. It is important to accurately document the number of vessels treated to ensure correct coding.
