2025 CPT code 37249

Transluminal balloon angioplasty, open or percutaneous, of each additional vein (except dialysis circuit), including all imaging and radiological supervision and interpretation.

This code is for each additional vein treated during the same session. Use modifier 59 if performing this procedure on a different anatomical site.

Modifiers may be applicable. Modifier 59 may be used to indicate a distinct procedural service.

Medical necessity for this procedure must be supported by documentation of the patient's condition and symptoms, such as chronic venous insufficiency, deep vein thrombosis, or other conditions causing venous blockage that significantly impairs blood flow. The documentation should also explain why less invasive treatments are not suitable.

The physician performs the procedure percutaneously or through an open incision, including inserting the catheter, inflating the balloon, and confirming the efficacy of the procedure using imaging guidance.

The physician performs transluminal balloon angioplasty on the initial vein and then uses 37249 for each additional vein treated during the same session., During a surgical procedure, a patient is found to have stenosis in several veins.Angioplasty is performed on the first vein (37248) and code 37249 is used for each subsequent vein treated., A patient with chronic venous insufficiency has several blocked veins in their arm. The physician performs angioplasty using 37248 for the initial vein and 37249 for each additional blocked vein treated in the same operative session.

Documentation should include details of the initial vein treated (37248), each additional vein treated (37249), the approach used (open or percutaneous), imaging guidance used, and the location and severity of the blockage in each vein.

** Code 37249 is an add-on code for 37248 and should not be reported alone. Always check for NCCI edits before billing multiple codes together.
