An EGD with biopsy is performed to evaluate for GERD and Barrett's esophagus., A patient with a history of peptic ulcers experiences recurrent abdominal pain. An EGD with biopsy is performed to assess for active ulcers and H. pylori infection., During a routine EGD, a suspicious polyp is discovered in the duodenum. A biopsy is taken to determine if the polyp is benign or malignant.

Documentation should include the indication for the procedure, the findings during the examination (including location and description of any abnormalities), the number and location of biopsies taken, and any complications encountered.
